The following documents must be provided to register all Pre-K through grade 12 students.
DOCUMENTS TO BE PRESENTED:
The following documents must be presented at the time of registration. Please contact your assigned school to schedule an appointment to register your child.
- Original Birth Certificate with raised seal or Official Passport.
- Immunization record.
- Proof of current physical exam.
- Original signed deed or lease.
- Original current PSE&G bill. One other form of identification with a South Brunswick address.
(e.g.: current driver’s license, pay stub, etc.)
- If applicable, custody, guardian, or caregiver documents.
Statement of Assurance will be provided by the South Brunswick School District for the parent/guardian’s signature.
**If you are registering under one of the provisions listed below, please contact Patricia Gable at 732-297-7800 ext. 3107 in the Office of Assessment and Instructional Support at the Board Office to schedule an appointment to register your child.
- Families who are living with a South Brunswick resident:
- Original Birth Certificate with raised seal or Official Passport.
- Immunization record.
- Proof of current physical exam.
- Original signed deed or lease of the South Brunswick resident.
- Notarized letter, listing by name all of the registrant’s family, from the South Brunswick resident that certifies the registrants are living in their residence.
- Registrant must provide three documents that list the South Brunswick address (example current driver’s license, pay stub, etc.)
- If applicable, custody, guardianship, or caregiver documents
Statement of Assurance will be provided by the South Brunswick School District for both the parent/guardian and the South Brunswick resident for signature.
- Families enrolling on an affidavit status:
- The student must be living with a district resident for reasons other than obtaining a free education in the South Brunswick Schools.
- The parent of the student must complete an affidavit attesting that the parent is unable to care for the child due to a family or economic hardship.
- The South Brunswick resident must complete an affidavit attesting that the child is living with them.
- Original Birth Certificate with raised seal or Official Passport.
- Immunization record.
- Proof of current physical exam.
