Sports Physical Policy
Effective 1/1/2008
The NJ Dept. of Education has revised their athletic physical forms. All forms submitted after January 1, 2008, must be completed on the new revised forms.

Please complete the following:

Form 1 : Student Sports Emergency Information (This form is 3-ply, if you are downloading this form you must submit in triplicate.) CLICK HERE TO DOWNLOAD FORM 1

Part A - Health History Questionnaire

Part B - Physical Evaluation Form

Both Form 1 (in triplicate) and Form 2 must be submitted together to the Health Office 3 weeks prior to the first day of tryouts. All forms will be sent to the school physician for clearance. If the student’s current physical will expire during this 3 week period call the Health Office.

If a student has a physical on file in the Health Office that is within one year of the start of the sport, just complete Form 1 (in triplicate) and Form 2, Part A Heath History Questionnaire. Both of these forms must be completed for each sport season.

All forms must be dated within 60 days prior to the first day of tryouts. All spaces must be completed. All questions answered yes on the Health History must be explained. Forms missing information will be returned to the student for completion/correction.

REMINDER:
If you already have a current physical on file, you must still complete Form 1 "Student Sports Emergency Information Form" and Part A of Form 2 the "Health History Questionnaire."


If your physical exam expires during the sport season, take the completed Part A (Health History) and Part B Physical Evaluation form to your doctor for review and completion.
