Sports Physical Policy
Beginning with 2010-2011 Fall Sports Season

**Please Note: Clearance requirements for students who have Diabetes, a Seizure condition, use an EpiPen or Inhaler**

All Students: The following Sports Forms must be submitted to the Health Office no earlier than 60 days prior to and no later than 3 weeks prior to the first day of tryouts.

Student Sports Emergency Information (Page 1) - parent permission to participate in sport - COMPLETED IN TRIPLICATE FOR EVERY SPORT
Part A - Health History of student completed by parent - COMPLETED FOR EVERY SPORT
Part B – Physical Evaluation completed by physician - valid for one year

Guidelines IF you need a Physical Evaluation, or need your doctor to complete Part B prior to the start of the sport: IF your Physical Evaluation (Part B) is on file in the Health Office and is within one year of the start of the sport: IF your Physical Evaluation expires during the sport season: IF your physical expires before the start of tryouts, but after the date due to the health office call your nurse’s office for instructions.

These forms are valid for the current school year only and must be completed and submitted to the Health Office at the beginning of the school year or when first diagnosed.
