HEALTH ENROLLMENT



School health services are maintained to appraise and promote the health of the students and school personnel.

In order for your child's school health record to be complete, the following information must be provided prior to your child entering school: In addition, if your child has asthma, the following is required: If your child is to receive medication during school hours, a completed Medication Order Form must be completed by a licensed health care provider and signed by the parent or guardian.

If you are transfering from another county within New Jersey or from another state, your child might have to have a Mantoux Test. This test may be administered by your health care provider or the school nurse. Please contact Mrs. Kramer, RN to inquire if this specific test pertains to your child.
