HEALTH ENROLLMENT
School health services are maintained to appraise and promote the health of the students and school personnel.
In order for your child's school health record to be complete, the following information must be provided prior to your child entering school:
If you are transfering from another county within New Jersey or from another state, your child might have to have a Mantoux Test. This test may be administered by your health care provider or the school nurse. Please contact Mrs. Kramer, RN to inquire if this specific test pertains to your child.

HEALTH ENROLLMENT School health services are maintained to appraise and promote the health of the students and school personnel.
In order for your child's school health record to be complete, the following information must be provided prior to your child entering school:
- Physical examination by a licensed health care provider and a completed Physical Exam Form
- Completed Health History Form
- Allergy Questionnaire (if applicable)
- Asthma Action Plan Form completed by a licensed health care provider.
If your child has asthma, it is now required by law (NJAS 8:61-1, 1(f) ) that every child with asthma has an Asthma Action Plan completed by their health care provider and kept o file in school. This form must be renewed every school year.
