Health Services Provided at Indian Fields

Each school's Certified School Nurse provides routine health screening and emergency care, as well as information and counseling for students and families concerning disease prevention and health related issues. The Certified School Nurse manages the school's health plan and notifies staff and patents about concerns related to the health of all students.

Annual Health Screenings

The certified school nurse conducts the following screenings:

Height, weight - All grades

Blood Pressure - K, 3, & 5

Vision - Grades K, 2, 4, 6, 8, & 10

Hearing - Grades K through 4, 6, 8, & 10

Scoliosis - Grades 5, 7, 9 and 11 (unless completed by private physician)

Physical Examinations

Students in the following grades MUST have a physical examination:

All students entering Kindergarten

All students new to the district

Students tryng out for athletic teams

Students participating in color guard, marching band or dance

Students applying for Working Papers

PHYSICAL EXAMINATION FORM REQUIREMENTS: Changes in the state school health code indicate a preference that the student's personal physician complete the required physical examinations. The district approved health form, completed by the physician, physician's assistant or nurse practitioner, must be submitted to the school nurse.
